Internship Weekly Log - you need to fill this up on a daily basis, every week and get your company supervisor to sign it off as a form of verification approval. 2. Appraisal Form - this is the final evaluation of your placement from your company supervisor. ONLY HAND THIS TO YOUR SUPERVISOR 2-3 WEEKS BEFORE YOU FINISH.
